• Range 82 ($66,331.20 – $111,667.20 Annually).

Human services field includes the following disciplines: Social Work, Psychology, Criminal/Juvenile Justice, Special Education, Sociology, Speech Education, Rehabilitation, Counseling, Speech Pathology, Audiology, Nursing, Physical or Occupational Therapy, and any related academic disciplines associated with the study of Human Behavior, Human Skill Development, or Basic Human Care Needs.

• Maintains working knowledge of laws, rules, and regulations from the Centers for Medicare and Medicaid Services (CMS), the Alabama Medicaid Agency (AMA) and the Alabama Department of Mental Health (ADMH) relating to Home and Community Based Service Waiver providers, specifically ID/LAH waivers, and works collaboratively with the Waiver, Quality Assurance, Fiscal, Community Service, Certification and Behavioral Health Directors to ensure adequate compliance and other relative staff necessary to manage and provide oversight to the Division’s HCBSW Support Coordination Services.
• Responsible for the coordination and monitoring of service coordination activities for divisional waivers and waiting lists and developing reporting activities to measure performance to include waiver performance measure requirements.
• Ensures compliance of service coordination activities with community contracts, the ADMH/Division of DD Community Standards, Medicaid, and waiver requirements.
• Oversees and maintains the Medicaid-approved statewide certificate training for new service coordinators and specialized training modules for continued education/training and competency for service coordinators.
• Maintains a training and certificate documentation database for all certified service coordinators.
• Provides supervision to the DD Call Center Coordinator and Waiver Intake activities from point of entry to the Wait List to ensure that the Call Center functions complement and enhance service coordination efforts at the state and local levels.
• Works closely with Community Services Directors and their staff to develop monitoring functions that assist the Division of DD with meeting waiver requirements, SC Staff development, and utilization review strategies to include those related to Self-Directed Services.
• Serves as a point of contact and liaison with the Alabama Medicaid Agency on all Division of DD targeted service coordination matters and serves on necessary internal and external workgroups that have an impact on related services.
• Initiates and participates in community outreach to support Alabama Department of Mental Health and ensures they are represented in a manner that promotes the mission, vision, and values.
• Knowledge of case management in health or waiver service systems environment.
• Knowledge of state and national trends in the provision of services and supports for targeted populations (including IDD).
• Knowledge and understanding of the care management core competencies, policies, procedures, and guidelines.
• Knowledge of related federal and state laws, rules, and regulations, to include Medicare, Medicaid, and Social Security benefit programs.
• Knowledge of targeted service coordination or support coordination through a waiver service.
• Knowledge of the Division of DD HCBS waiver requirements for access to services or requirements by other state/federally funded programs where case management is required.
• Knowledge of eligibility Intake process and requirements to include eligibility for waiting list.
• Ability to exercise the judgement, decisiveness, and creativity required in situations involving the direction, control, and planning of an entire program or multiple programs.
• Ability to identify learning opportunities and develop curriculums to ensure a knowledgeable support coordination workforce (internal and external stakeholders).
• Ability to establish and maintain effective working relationships, and collaborate with a variety of individuals, both internally and externally.
• Ability to empower a team and maintain a positive teamwork atmosphere.
• Ability to plan, prioritize, and organize work efficiently; work under pressure and deadlines, analyze problems, propose reasonable solutions, make logical decisions, carry out decisions made, and follow up with feedback as appropriate.
