About The Toolkit
This toolkit is aimed at providing states information and training material to ensure state staff, supports coordinators, and direct support professionals understand how Medicare and Medicaid programs can be complementary, as well as support the education and guidance of individuals and families to navigate dual eligibility enrollment and ongoing benefits management. The work of the TA cohort set the stage for policy and program refinement through the development of this NASDDDS I/DD Dual Eligibility Toolkit.
NOTE: All PDF documents in this toolkit have a Certificate of 508 Compliance: Communications Tools and Policy Tools. All customizable documents and PowerPoint Documents are not 508 compliant and must go through your own remediation process.

- A tool for state I/DD leaders and program managers that describes the various Medicare & Medicaid data sources available to states. The tool describes the data sources, their purposes, formats, frequencies, access methods, and weblinks.
- State Medicaid Agency Contract (SMAC) National Review and I/DD Language Examples
- A national summary of SMACs that include specific references to and/or provisions for individuals with I/DD. This is a reference guide for states improving opportunities for people with I/DD who are dually eligible for Medicare & Medicaid through the SMAC.
- State Policy Considerations: Dual Eligible Integration Opportunities to Improve Support for People with I/DD
- An outline of key areas state I/DD authorities may consider in working with the state Medicaid agency to advance education, increase awareness, and improve care coordination for individuals with I/DD who are dually eligible for Medicare & Medicaid. These strategies, successfully implemented in other states, can serve as future goals to improve alignment of services and supports for individuals with I/DD who are dually eligible. Additionally, these approaches may be adapted for the broader dually eligible populations across the state.
